Payment for services is always due at the time of service, and may be made by check, Paypal or credit card (including HSA cards, when applicable). All patients are required to keep a current credit card on file, which will be charged following each billable service, unless an alternate form of payment is provided at the time of service.
My practice is out-of-network for insurance. I will provide you with statements that include all information needed to seek out-of-network reimbursement from your insurance carrier, if included in your insurance plan. Out-of-network reimbursement rates vary based insurance carrier, plan and service provided.
